Marketplace preference for greater co-pay offerings in plans is combined with daily hospital rates for Inpatient and Outpatient surgeries plus professional services to reduce employee and family costs while managing claim invoicing. Introducing a daily health care plan that is easy to understand, utilizes cost-controlling co-pays plus Teledoc telemedicine to reduce cost anxiety and handle unforeseen care issues.

Providers include Primary Care Physician, Pediatrics, Internal Medicine, Specialists, and Urgent Care. Inpatient and Outpatient Hospital, Surgical, and Professional Services payments utilize daily rates. Pharmacy needs are provided in the plan by ServeYou Rx and their nearly 70,000 nationwide pharmacies. Mental Health benefits included.

SB/A CoOp Core Health and  Freedom Icon I plans offer zero deductibles and 50/50 coinsurance, or copays for simpler office diagnosis.

The Dental Plans

Affordable and comprehensive Dental/Vision coverage is now available with SB/A Freedom Icon’s new dental and vision plan options. We offer a traditional PPO model using DenteMax’s nationwide dental professionals  – the plan covers exams, X-rays, fillings, crowns, bridges, and orthodontia, all the services members require.

For an even more affordable coverage option, we offer a reimbursement plan that covers dental and vision services for up to $1800 per year.  Patients can use the dental and vision providers of their choice and be reimbursed amounts based on co-insurance levels.  This is the dental and vision program that employers and members have been asking for – it is affordable and allows members to keep their dental and vision providers that have come to trust.

    Employer Enrollment Packet with DocuSign (see button below)
  • Step 2 – Loomis (TPA) will set up your enrollment portal account and email logins to you
  • Step 3 – Fill out the Employee Eligibility Census
  • Step 4 – Login to your Employer portal at Loomis and securely upload your completed Census file
