Our Core Foundation is the Small Business/Agency Cooperative, Inc. – a Colorado Non-Profit Cooperative Corporation. The SB/A CoOp is a lawful business under the Colorado Revised Non-Profit Act able to operate in all 50 states and U.S. territories as a collective legal agent for any individual, firm association, corporation, or other legal entity. The SB/A CoOp promotes, facilitates, and arranges for employer/employee benefits and/or other services between and among and between the members of the Cooperative all on an agency cooperative basis.
In short, the SB/A CoOp creates ACA Qualified ERISA Employee Benefit Plans for employers with 2 or more enrolled employees. We call them the FMA Freedom Select Plans. These plans have the following qualities:
- Partially Self-Funded, Level-Funded ERISA compliant employer plans
- Affordable Care Act (ACA) qualified – 100% unlimited coverage for Minimum Essential Coverage benefits
- Plans cover inpatient/outpatient hospitalization, physician professional services, prescription drugs, behavioral health care, chiropractic care, medical imaging & x-ray, emergency room & urgent care services, and so much more
- Lower Annual Maximum Benefit Plans with catastrophic buy-up coverage options are offered through Free Market Administrators LLC., our Plan Administrator. Benefit add-ons of $150,000, $250,000, or $500,000 per year – give employees have the flexibility to select/purchase coverage levels suitable for their individual needs
To enhance the FMA Freedom Select Plans, we partnered with the Alliance for Shared Health (ASH) who created the ASH Catastrophic Plans which will allow an employee (or employer) to purchase an additional $150,000, $250,000, or $500,000 per year of coverage for peace of mind. The ASH Catastrophic is a great compliment to the FMA Freedom Select health care and medical benefits plans. The coverages of FMA Freedom Select Plans and ASH Catastrophic Plans are fully administered by Free Market Administrators LLC., (FMA) for billing, membership, claim adjudication, and customer service.
